Senior Care Worker/Care Coordinator

Job Description
Duties and Responsibilities:

Assisting clients with daily tasks such as washing, dressing, grooming, and using the toilet. This is a core part of the job and requires empathy, dignity, and a focus on maintaining the individual's independence.

Must have access to your car and be able to drive to service user's home and take service user out where necessary.

Assist service users with medication administration, and monitoring for any changes in a client's physical or mental health.

Helping clients move around safely, this may involve using specialized equipment like hoists or walking aids.

Assist with meal preparation and ensuring adequate hydration.

Provide emotional support and engage service users in meaningful activities.
-
Support with light domestic duties and ensure a clean and safe environment.

Assist with the induction and training of new Care Workers.

Monitor the performance of Care Workers and provide constructive feedback.

Conduct regular spot checks and observations of care delivery.
-
Assist in the assessment of new service users and the development of person-centred care plans.
-
Developing, implementing, and regularly reviewing individual care plans to ensure they are tailored to each person's specific needs and preferences.

Participate in multidisciplinary team meetings and contribute to care reviews.
-
Maintain effective communication with service users, their families, and advocates.

Liaise with other healthcare professionals.
-
Report any safeguarding concerns to the appropriate authorities.

Adhere to all health and safety policies and procedures.

Overseeing and monitoring a team of care assistants or support workers, providing guidance, and acting as a positive role model. This includes taking responsibility for the service and the team during their shift.

Providing guidance and support to new and junior staff.

Conducting formal supervision sessions and spot-checks to ensure care is being delivered to the required standards.

Maintaining meticulous records and documentation, reporting any incidents or concerns, and communicating effectively with clients, their families, and other healthcare professionals (e.g., GPs, social workers).

Taking charge during emergencies and providing guidance and support to the care team.
